Loaner Uniform Agreement Terms and Conditions

By clicking the checkbox, If approved, I agree to the loan of this property to the players listed above. I agree to see that the property loaned to the player is properly cared for and maintained. I further agree that the uniform is received in good condition. In the event that the uniform is damaged beyond normal wear and tear, lost or stolen, or is not returned for any reason, I agree that I am financially responsible and will be required to pay the ‘Villains Basketball Organization’ for the full cost of the repairs or replacement. The assessed value and/or damage shall be determined by the ‘Villains Basketball Organization’ and by the maker of the uniform. I further agree to return this uniform at any time upon request of the organization.
