UnIform Loan Agreement Form

Players who participate in the ‘Villains Basketball’ Program may apply to receive a loaner uniform upon request, which, if approved, shall serve as a placeholder until the player submits and receives their own personal uniform set. All items loaned will be collected upon receipt of the players personal uniform set or at the end of the basketball season. Please take good care of the loaner gear, otherwise the parent/legal guardian of the player will be charged a replacement fee (retail value) for any damaged or lost item(s).

Shooting Shirts & Practice Jerseys

We highly recommend you purchase your own Shooting Shirt and/or Practice Jerseys since our supply is very limited. However, if you need a loaner practice jersey, please indicate below.

Please mark your practice jersey sizes below. All uniforms (jersey + short) and practice jerseys will be delivered to the player during practice, game or will be arranged with parent or legal guardian to be picked up.
